What is primary care?

Primary care is your main, ongoing health care. Your primary care provider gets to know you over time, helps you stay well, treats common health problems and manages long-term conditions.

At Revitalized Wellness, primary care is provided by nurse practitioners: advanced practice nurses with graduate training who diagnose, treat and prescribe. Your primary care provider can coordinate with the rest of our team when you need more support.

Who it may help

  • Adults looking for a regular primary care provider
  • People living with ongoing conditions such as high blood pressure, diabetes or high cholesterol
  • Anyone due for a checkup, preventive care or a Medicare Annual Wellness Visit
  • People who need prescriptions, refills or lab work followed over time

  • Preventive care and screenings
  • Lab work by appointment, point-of-care tests and EKGs, when ordered
  • Prescriptions and refills, when clinically appropriate
  • A plan you make together with your provider, with follow-up
