I started Revitalized Wellness in 2020, when telehealth made it possible to care for people from anywhere. At the same time, I was doing telehealth for people in recovery, and I kept seeing the same gap. My patients had no primary care. So I built it, and that work grew into our clinic in Eugene, Oregon, where people get primary care, medication treatment for substance use disorder and mental health care in one place. In September 2025, we opened our clinic in Red Oak, Iowa.
I’ve worked in settings where every visit had a clock on it and there was no time to talk about the things that change a life, like food, sleep, movement and hormones. I wanted a practice where we take the time, where medication is one of the tools we use, and where you leave with a plan that fits your life.
I know this one from the inside. When I hit menopause, the weight came back, even though I was eating keto. I’ve been judged for my body, and for a while I pulled back from sharing anything at all. That’s part of why our weight and hormone care looks the way it does. We look at the whole picture, and nobody gets shamed here.
Most of the people we see have carried a lot. They’ve been rushed, judged or turned away. When you walk into our clinic, I want it to feel calm. I want you to feel welcome, heard and safe, like you’re home.
It’s all about the progress not the perfection.

Marta Shevchenko
MSN, APRN, PMHNP-BC
Psychiatric-Mental Health Nurse Practitioner
States: Oregon
Oregon telehealth only. Patients may also join her telehealth visits from our Eugene clinic, with support from our clinic staff.
A board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP) with a strong foundation in both inpatient and outpatient mental health care. I work with clients who are navigating challenges like anxiety, depression, trauma and other mental health issues. As an immigrant myself, I understand how complex it can be to balance cultures, identities, and the pressures of adapting to a new environment. My background as a nurse and nurse practitioner has given me the privilege of working with people from a wide range of cultural and ethnic backgrounds—and that has shaped my practice in a deep and meaningful way. I’m bilingual, and speak fluent Polish and I also understand and speak some Spanish and Russian. I believe language and cultural awareness are key parts of mental healthcare. My approach is supportive, collaborative, and focused on helping my clients feel seen, heard, and empowered to make meaningful change.

Holly M. Logan
MSN, ARNP, PMHNP-BC
Psychiatric-Mental Health Nurse Practitioner
States: Iowa
In person and by telehealth in Iowa.
Hello! My name is Holly Logan, and I am passionate about providing compassionate and comprehensive healthcare. With 20 years of experience as a nurse in a rural healthcare setting, I have witnessed firsthand the unique challenges that underserved communities face, particularly concerning mental and physical health. My journey in nursing has been both enriching and transformative, leading me to discover my true calling in psychiatric care.
I earned my Master’s Degree in Executive Nursing, which equipped me with valuable leadership skills. However, I soon realized that the traditional roles associated with that degree did not align with my personal values and aspirations. I felt a deep-seated need to connect with individuals on a more personal level, to dive into the intricacies of mental health, and to contribute to the well-being of my patients in a way that truly resonates with me. This realization motivated me to pursue a Post-Master’s Certificate as a Psychiatric Nurse Practitioner.
In my practice, I firmly believe that mental and physical health are inextricably linked. This understanding shapes my approach to patient care. I strive to establish a trusting provider-patient relationship where open communication is paramount. I recognize that every patient is unique, and therefore, I focus on educating and empowering them to make informed choices about their mental healthcare and treatment options. My goal is to create a supportive environment where patients feel valued, heard, and actively involved in their wellness journey.
When I’m not at work, I cherish spending time with my wonderful husband and our five children, as well as our precious grandchildren. Family is incredibly important to me, and I love creating lasting memories with them.
I am thrilled to combine my passion for mental health with my commitment to holistic care, and I look forward to being a part of your journey toward better health and wellness. Thank you for taking the time to learn more about me!
