Revitalized Wellness Foundation exists to support patients and communities through:

• Mental health and counseling support

• Substance use recovery services

• Medication-assisted treatment support

• Women’s health and hormone care education

• Chronic disease prevention

• Remote patient monitoring support

• Transportation and care access assistance

• Community wellness education

• Support for uninsured and underinsured patients

We believe healthcare should not stop because someone cannot afford a visit, transportation, monitoring equipment, labs, or basic wellness support.

 

Why We Exist

Many patients face barriers that make it difficult to receive consistent care, including:

• Lack of insurance

• High deductibles or copays

• Transportation challenges

• Food insecurity

• Mental health needs

• Substance use recovery needs

• Limited rural access to care

• Difficulty affording labs, medications, or monitoring devices

The Foundation helps bridge these gaps so patients can stay connected to care and continue moving toward stability, healing, and long-term wellness.

 

Programs We Support

• Patient Care Assistance Fund

  • Helps eligible patients access medical visits, behavioral health appointments, labs, medication support, and care coordination.

• Recovery & Behavioral Health Support

  • Supports patients working toward recovery from substance use disorders and those needing mental health services.

• Women’s Wellness Access Program

  • Provides support for women navigating hormone changes, perimenopause, menopause, PCOS, insulin resistance, and preventive health needs.

• Rural Health Access Initiative

  • Expands care access for patients in rural and underserved communities through telehealth, remote monitoring, education, and community partnerships.

• Remote Patient Monitoring Support

  • Helps provide blood pressure cuffs and monitoring tools for patients with chronic conditions such as hypertension and cardiovascular risk.

• Community Education & Prevention

  • Offers education on chronic disease prevention, mental health, nutrition, substance use recovery, women’s health, and wellness.

Our Connection to Revitalized Wellness

Revitalized Wellness Foundation works alongside Revitalized Wellness clinic to support whole-person care. While the clinic provides medical and behavioral health services, the Foundation helps reduce barriers that prevent patients from accessing and continuing care.
